Job summary

Responsible for timely, accurate and comprehensive abstraction of hospital services from the medical chart/record, utilizing appropriate CPT-4 procedure and ICD-9 CM diagnosis codes.

Summary of essential job functions Reviews medical record documentation to identify all services provided by the hospital. Assigns appropriate CPT-4 procedure code(s) to accurately report the physician services provided to patients. Assigns appropriate ICD-9 diagnosis code(s) to accurately support the need for each service Responsible production and delivery of quality services within the agreed turnaround time Audit of Coded charts Responsible for the Training of the fresh Coding staff.
